Cranberry extract (Vaccinium macrocarpon) standardized to A-type proanthocyanidins (≥36 mg PAC/dose) inhibits uropathogenic E. coli (UPEC) adhesion to bladder epithelium via antiadhesive mechanisms, independently of urine acidification. Meta-analysis of 34+ RCTs demonstrates 25–35% reduction in recurrent UTI incidence, particularly effective against UPEC-mediated infections. Active constituents also include anthocyanins, flavonols, and benzoic acid derivatives with antioxidant properties. INTERACTIONS: may potentiate warfarin/anticoagulants through CYP2C9 inhibition — monitor INR in anticoagulated patients; avoid in individuals with salicylate sensitivity. PAC content and bioavailability vary significantly between products.
