A 2025 meta-analysis of 6 RCTs (428 participants) found hawthorn statistically significantly decreased systolic blood pressure by 6.65 mmHg. Proposed mechanisms: vasodilation, increased coronary blood flow, mild positive inotropy. Generally well tolerated. Caution with cardiac glycosides (digoxin — toxicity risk), antihypertensives (additive hypotension), and phosphodiesterase inhibitors.
