Overview

Modafinil is a eugeroic (wakefulness-promoting) pharmaceutical, distinct from classical stimulants. Its primary mechanism involves inhibition of dopamine reuptake transporters and activation of the orexin/hypocretin system, producing sustained wakefulness without the adrenergic surge of amphetamines. Approved for narcolepsy, obstructive sleep apnea, and shift work sleep disorder. Used off-label for cognitive enhancement, fatigue reduction, and jet lag adjustment. Half-life is 12–15 hours. Unlike methylphenidate, it has low abuse potential and minimal crash effect. Relevant stack interactions include CYP3A4 induction (reduces levels of hormonal contraceptives, some supplements); it may also reduce efficacy of cyclosporine. Cognitive enhancement effects are most pronounced in sleep-deprived individuals; evidence for enhancement in non-sleep-deprived populations is more limited.
