Tesamorelin
Overview
Tesamorelin is a synthetic analogue of endogenous GHRH comprising all 44 amino acids plus a trans-3-hexenoic acid modification that extends plasma half-life. FDA-approved (Egrifta®, BLA022505) for reduction of excess visceral abdominal fat in HIV-infected adults with lipodystrophy at 2 mg (2000 mcg) daily subcutaneous injection. Phase III trials (PMID 21283099) demonstrated 15–20% visceral adipose tissue reduction versus placebo. IGF-1 rises within physiological range. The PRIME study (NCT02572323) additionally showed improvements in verbal learning and memory. INTERACTION NOTE: GH elevation may mildly increase insulin resistance — glucose monitoring is clinically recommended during use. Contraindicated in active malignancy and pituitary tumors.
