Overview
D-Ribose (C5H10O5, CID 5779) is a naturally occurring pentose sugar and rate-limiting substrate for adenosine nucleotide (ATP/ADP/AMP) synthesis in high-energy tissues. Theoretical premise: under ischemia or metabolic stress, ribose supplementation accelerates ATP pool replenishment by bypassing the slow pentose phosphate pathway. Small trials reported modest improvement in exercise tolerance in cardiac patients; however, an RCT found no significant preservation of high-energy nucleotides or cardiac function even combined with creatine. Evidence remains limited to mechanistic studies and small case series. Well-tolerated at 5–15 g/day. INTERACTIONS: potential mild hypoglycemic effect in diabetics (monitor blood glucose); may transiently lower blood sugar — caution with antidiabetic medications. No major drug interactions.
