L-carnitine is a quaternary ammonium compound (C7H15NO3) synthesized from lysine and methionine, essential for transporting long-chain fatty acids into mitochondria for beta-oxidation. Used for cardiovascular support, exercise performance, and cognitive function. A 2024 review raised safety concerns: gut microbiota convert L-carnitine to TMAO, linked to atherosclerosis risk. Generally well tolerated at standard doses; GI side effects at higher doses. Safety at high doses "remains an issue for scientific investigation."
