Plant Sterols

Metabolic Evidence: strong

Overview

Plant sterols/stanols (β-sitosterol dominant, CID 222284; also campesterol, stigmasterol) are structurally similar to cholesterol and competitively inhibit intestinal cholesterol absorption via ABCG5/ABCG8 transporter competition. Doses of 2–3 g/day produce consistent LDL reductions of 6–15% across 28+ RCTs — evidence level is strong. Endorsed by EAS and ACC/AHA guidelines for dyslipidemia management. IMPORTANT INTERACTIONS: reduce absorption of fat-soluble vitamins A, D, E, K — supplementation of fat-soluble vitamins recommended with long-term use; additive LDL-lowering with statins (safe combination); may reduce absorption of lipophilic drugs (cyclosporine, digoxin) — take 2h apart. Individual response influenced by ABCG8/ABO genetic variants.
