Piracetam
Overview
Piracetam (2-oxo-1-pyrrolidineacetamide) is the original racetam nootropic, developed in the 1960s. It is thought to modulate AMPA receptors and enhance membrane fluidity. Meta-analyses show modest cognitive benefits in older adults with impairment; evidence in healthy individuals is inconclusive. A 2024 systematic review (886 patients) found no definitive effect on memory function. Typical studied doses are 800–4800 mg/day.
Dosage
800 mg · Morning · Can take on empty stomach
Key Interactions
6 verified interactions found
Both piracetam and ginkgo biloba exert mild antiplatelet and anticoagulant effects. Their combination may increase the risk of bleeding, particularly in patients on anticoagulant therapy or before surgery.
Use with caution. Avoid in patients on blood thinners (warfarin, aspirin, clopidogrel). Discontinue at least 2 weeks before surgery.
View details →Piracetam and noopept share overlapping mechanisms (AMPA receptor modulation, cholinergic enhancement). Combining them provides no clear additive benefit and increases the demand for choline supplementation. Excessive glutamatergic stimulation is a theoretical risk.
Avoid combining without clear clinical indication. If stacking, ensure adequate choline supplementation.
View details →Caffeine and piracetam are sometimes co-administered in nootropic stacks. Caffeine provides acute stimulation while piracetam may enhance long-term cognitive function. However, caffeine can increase anxiety and counteract some of piracetam's calming cognitive effects.
If combining, use moderate caffeine doses (≤100 mg). Monitor for anxiety or sleep disruption.
View details →Piracetam increases the utilization of acetylcholine in the brain, which can deplete choline stores. Co-supplementation with choline is widely recommended in nootropic practice to prevent headaches and maintain cognitive benefits.
Stack piracetam with a choline source (choline bitartrate, alpha-GPC, or citicoline). Standard ratio: 4:1 piracetam to choline by weight.
View details →Alpha-GPC is a highly bioavailable choline donor and the preferred pairing with piracetam in nootropic stacks. It replenishes acetylcholine precursors that piracetam's mechanism depletes, and may amplify cognitive benefits.
Combine 800 mg piracetam with 300 mg alpha-GPC, taken in the morning.
