Tribulus terrestris contains steroidal saponins including protodioscin (C51H84O22). A meta-analysis of 8 RCTs found significant improvement in erectile function (IIEF-5: +4.21 vs. placebo) but no significant increase in testosterone levels — the claimed testosterone-boosting effect is not supported by meta-analytic evidence. Benefits for erectile dysfunction operate through non-testosterone mechanisms. Generally well tolerated. No evidence for athletic performance enhancement. Contraindicated in hormone-sensitive prostate cancer. Monitor blood glucose with antidiabetics.
