Overview
Andrographolide (C20H30O5) is the primary bioactive diterpene from Andrographis paniculata, acting via NF-kB inhibition with antiviral, anti-inflammatory, and immunomodulatory effects. Used for upper respiratory infections. A meta-analysis found serious adverse events from oral Andrographis extracts are extremely rare (0.02 per 1,000 patients). Injectable andrographolide derivatives carry 4–5% ADR rate with anaphylaxis risk — oral standardized extracts (≥10% andrographolides) are generally safe. Contraindicated in pregnancy. Caution with autoimmune conditions and immunosuppressants.
