Overview
L-glutamine (C5H10N2O3) is the most abundant free amino acid — conditionally essential under catabolism, trauma, and intensive exercise. Key roles: enterocyte and immune cell fuel, glutathione precursor, nitrogen transport. A meta-analysis of 27 studies showed perioperative glutamine reduced infectious complications (RR 0.36), non-infectious complications (RR 0.32), and hospital stay by 2.3 days in colorectal cancer patients. Caution: contraindicated in hepatic encephalopathy (ammonia production). High-dose IV glutamine in critical illness may increase mortality (REDOXS trial).
